The TPE in Sepsis Story
Over the years many centers have used TPE in cases of refractory sepsis with associated organ failure. Numerous small studies have been performed and several in the pipeline. To date, the data has been somewhat mixed with a trend towards improved survival and organ function. One of the difficulties is identifying those that have the sepsis response phenotype most conducive to responding to plasma exchange. Other complicating questions include timimg and type of replacement solution. To complicate things further, there are two available modes that can be used for plasma exchange. One is centrifugation and the other is a filtration method using a highly porous membrane. Now in the mix is cytokine absorption filter. All these questions are unlikely to be solved in standard RCT designs.
To counterbalance this deficit, it is PFSO's mission to build a registry for complex data mining that may shed light on many of these questions and provide a platform for improved trial design. So goes the tissue bank so this can also be tapped into to better identify phenotypes most responsive to TPE.
